Referral Program at Acorn Dental Associates

Thank you for referring your family, friends and coworkers to Acorn Dental. We are committed to our patient's dental health and strive to provide excellent care and service. Perhaps you know others like yourself, who would appreciate our type of office.

Referral Program Guidelines:

1. New patient or patient of record must tell Acorn Dental staff who referred them or who they referred.
2. Only those patients 21 years of age and older are qualified for this program.
3. There are no time limits on this program.
4. This program stands alone and cannot be combined with any other program at Acorn Dental.
5. The Patient Rewards Program has four levels:
   • 1st referral - Two tickets to the Ritz Theatre or $25 credit at Acorn Dental
   • 3rd referral - $50 to Barnes & Noble or a Sonicare Toothbrush
   • 5th referral - $100 shopping spree at The Promenade or $100 to The Pacific Grill Restaurant
   • 10th referral - In-office whitening or $500 to Best Buy
